After trying countless styles of reading logs, this set-up hit a home run with my students. It is just the right amount of "cutesy" and middle school-cool. It's easy to use and visually appealing to those students who thrive on structure. It allows for easy monitoring of reading that happens (or doesn't happen) at home. Print it out, copy back-to-back, hole punch and stick it in their binders!
Food For Thought: make each day of the month worth 2-3 points. At the end of the month, all you need to do is count the days their log was completed and BAM!, you just got yourself an easy grade.
