This graphic organizer asks students to give information about themselves in terms of numbers. How a teacher expects them to do that is up to them. Examples could be:
Just writing the numbers
Using order of operations to create number sentences
Using complex functions for upper high school and college students.

There is also a splat in the middle for students to draw a picture of themselves. The entire thing is black and white to allow for coloring, and the border is simple to allow for cutting out and pasting on colored paper.
