I use this questionnaire on the first day of school every year; it makes a great entry slip for the first 15 minutes or so. It helps me get to know a little bit about the kids' living situations, the technology they have accessible, and their math teachers from the previous year. I also ask them to share with me if there are students in the class they can't work with or will work with really well. Finally, there's a space for the students to tell me anything about themselves, about their summers, or ask me any questions that haven't been answered yet. I often find out important information, like students who feel they need to share that they are ADHD, have had a parent pass away recently, or like/hate math. It's a great ice-breaker that I love to use every year.
Although I designed it for 8th grade math, it could be easily modified for another grade level. This version is in Microsoft Office so that it can be edited.
