I created this incentive slip to go with my multiplication mon"star"s spinner games, but it can be used with any math fact program you have in place to encourage extra fact practice at home.
To use these incentive slips, I send home a blank page with each student at the beginning of the year. Once they have studied their math facts for a least ten extra minutes, they bring in a completed slip. I have them turn the completed slips into a bucket and once there are enough links in the bucket, I staple them together in a chain. Once the chain reaches a certain length, determined by you, the class receives a reward like a special snack during math or if it is closer to a holiday party, a pizza lunch.
I have included in this download a note to you on how I use the incentive slips, the actual incentive slips themselves, and labels for storing the incentive slips in your classroom.
