This is the visually friendly math test you never had time to create. There are typically 6 questions per page on one or more concepts. I clearly indicated those at the top of each page. You can replace a whole final exam or use chunks to assess skills and progress.
I like giving one page a day to keep students from getting over whelmed. Like items are kept together so students can focus. This test is not a trick test. I set it up to support students and encourage their best possible performance.

The concepts include: addition, subtraction, rounding, place value, expanded notation, word form, standard form, multi-step word problems, GCF, prime factorization, factors, LCM, multiplication, division, square root, cube root, prime and composite.
Even though I use this in 9th grade the standards are as low as 4th.
