I used this sheet a TON at the end of the year to help students with test prep. The download is essentially a long chart with enough spaces for up to 100 questions. Although it is helpful for those EOY tests, it also can be used at any time during the school year for summative exams.
For each question, students must complete a box for:
1. Their work
2. Their answer
3. Their ranking of difficulty level
There are three difficulty levels described at the very beginning of the document with a symbol for each level.
I found the symbols really nice to have because it was easy to glance around and see how each student ranked each problem. It was also easy for review to just ask the students to look back through and go over any questions they ranked as a star with their partner. Much easier than thinking students will remember which ones were challenging!
Since the document table is multiple pages, the headers with what should go in each box, as well as an area for name, date, and class number, are at the top of each sheet so they don't forget what to do!
