This survey would be great to do at the beginning of the school year, and then again at the end. Student surveys can be filed away, and then pulled out to compare the progress of student attitudes toward math. It is especially effective if done before an intervention (growth mindset or Jo Boaler's "How to Learn Math for Students," online course).
