A friend of mine was lamenting how the speeding tickets for maths seemed to all be for elementary or lower school level. She wanted something more serious-looking and with specific options for code violations that she saw in her students' work, so I made her this form. It has two tickets per printable sheet, one issued as Pi and the other as a calculation related to Euler and exp(1). Fill in student name, grade, the date of issuance and the location of the offenses. Choose the violation type and fill in the points, and if you have a remedy or notes/comments, you can fill that in as well. I added rough Latin translations of the violation types to make the English teachers happy (if they are teaching Greek and Latin roots).

This actually applies to Common Core Standards for Mathematical Practice, and I related it to Middle School because that is the level that this friend teaches:
