This is a great tool that I have used over the past couple of years. A simple journal that I file in student portfolios (or stick in their books) that they use to reflect on their learning at the end of each week.
There are enough prompts for your students to reflect once a week, for 9 weeks.
This is used for a time for students to reflect on their learning, celebrations and where they are at. Being able to write it down ensures students are accountable for their successes and their learning; it also allows students to practice 'writing' maths. It encourages students to think critically about where they are, where they need to be and their next steps to get there.
Feel free to take the prompts and change it to a format that works for you.
