The mediation request is perfect for students who are triggered and/or provoked by a social conflict. It can be completed independently or it can be completed by an adult interviewing the child. When students are feeling angry, this form is the perfect intervention. In this manner you are able to receive all the information that an adult would need for an effective follow up intervention. This also helps the student develop self awareness and self management skills by reflecting on what they feel, think and need in order to move forward. The mediation request also serves as a report form to help students demonstrate decision making skills and responsible behaviors in the school setting. Once the students requests a mediation, the follow up is an opportunity for all parties to come together. This intervention is critical in assisting students in learning communication skills and conflict resolution skills.
