Need to hold yourself or your faculty accountable for making contact with mentored students? This is the resource you need! This spreadsheet checklist is fully editable in Microsoft PowerPoint and printable. It has space for twenty mentees.
My school recently developed a mentoring program that assigns 10-15 students to each faculty or staff member to be mentored. Each mentor is required to make contact with their mentees at least once a month. This checklist was designed to serve as a reminder for me to contact or meet with each child on a monthly basis, and could be used by teachers as a personal reminder or as an administrative requirement.
How to Use:
1. Type the names, teachers, and birth dates of the students you mentor in the first three columns on the left hand side. A sample is provided.
2. Print the checklist and keep it handy!
3. As the school year progresses, use the checklist to note each time you make contact with each child. Check the corresponding box for initial contact, one contact each month from September to April, and final (or follow-up) contact. A “contact” means a note, classroom visit, special event, or any contact approved by your administration. Enjoy!
