In order to determine the most effective way to review with students in preparation for their midterm exams (we called them "Common Assessments" at our school), I created this survey to gain some student input.
On the front, the semester is divided into two basic categories: grammar and vocabulary themes. Students rate within each category which topic they would like to review the most to the least (for example: I remember nothing about direct and indirect objects, so that would be 1).
On the back, there are two other types of questions. First, I ask students how they would like the review time to be structured. Then, students rate each topic on a scale of 1-5 on how confident they are about that aspect of the semester. This way, they are able to self-evaluate their personal studying needs on top of in class review.
I assigned this as homework as to make the most efficient use of class time possible.
