This is a simple activity which students can do independently or in groups to review the types of mirrors and lenses. They should cut apart the rectangles on the first page and then reassemble them onto a blank paper to match the type of mirror/lens to its picture, description of how it affects light, and example of where it is found. There are six categories: flat mirror, convex mirror, concave mirror, flat lens, convex lens, and concave lens. This would be a good activity to review these optical tools.
The answer key is included on the second page.
