One page has four homework slips. I usually print these on pink paper and refer to them as "Missing Homework Pink Slips." Each time a student misses his or her homework, I give them one. They immediately fill it out and return it. They write their name and date, identify the missing assignment, tell me the reason why they missed it, and provide their signature. This is a great way to teach the students accountability. It also helps me track their homework patterns. On parent-teacher conferences, I show the parents their child's pink-slips.
