This is a 5 page activity that can be used as a test for students with disabilities in an inclusive science class. It covers the basics of the human heart.
Students can make a booklet by coloring the cover when they need a break from the academic work.
Pages 2-3 are notes that students are to copy. These are simplified notes that are modified for students with disabilities to understand.
Page 4 is a puzzle. Students are to cut out the pieces of a picture, and then glue them together to make a heart. They can then color the heart.
Page 5 is the assessment. This is a fill in the blank activity with a word bank.
This is modified to about a Kindergarten level but can be used in any science class. I used it for grade 7.
