This is a one-page Monthly chart I developed for students with moderate to significant behavior concerns. With this form, I document how often a student gets in-class support, direct support, and/or group support. I also document absences, self-referrals to my office, behavior referrals, suspensions, and consultations with parents and teachers. At the end of the month, I calculate these "Indicators of Risk", and determine if the student is considered High-Risk, and which level of intervention / support is needed. I compare and keep the forms throughout the year as documentation of student behavior improvement or decline. I have included a sample chart for reference.
