For children with mood disorders, who have been homocidal or suicidal, it is important to keep a quantitative check-in and check-out to help with progress monitoring within the schools. This form was designed to be easy to use for teachers. Child must be able to read themselves. I use with the 5 point scale of 5 being angry or extremely sad and 1 being happy or feeling great. A picture chart of the 5 point scale is not included but can be if requested for extra small fee.

Waste of money, cheap but not a conversation of mood
