I use this sheet every morning with my special needs ID students. The sheet can be easily modified for all levels of students. Currently in my classroom I have students who can write their name and students who cannot. For students who can write it provides easy to follow boundaries. For the students who cannot write I print out the letters of their name and they are required to glue the letters of their name in the correct location. For their phone number and address students are required to locate their phone number or address in print and then glue it to the correct location on the form.
