PRODUCT DESCRIPTION

After returning from spring break it appears we have just turned a corner. I'm all of a sudden receiving student lists for next fall and scheduling move-up and transition meetings. As I am sure is the same for you, my students are my heart, my priority, my "babies". I want to make sure that their transition is as smooth as possible as they move on to the next setting which, for many of my kiddos is middle school. It is a little extra work, but as I have spent the last several years getting to know their quirks, likes and dislikes, what excites them and what continues to be a struggle, I want to give their next teacher all the notes I can to help my "babies" continue on a successful path. So I created this simple move-up form to cover the basics and can be used as a nice "cheat sheet" for students in new classrooms. It can also be a nice end of the year summary page to help you pick up where you left off come fall for those students you have returning to your classroom. I hope you find it useful!
