This is a basic template for helping students understand how the equation, slope/intercept, table, graph and a story are all different representations of the same situation. The one I uploaded was what I used to review linear vs. exponential change with a senior math class. However, it can be all linear if you are working in a linear unit, all exponential, quadratic, whatever. I uploaded it in Microsoft Word so you can change it however you see fit. The goal for the students is to be given one piece (like a story) and be able to fill in all the other pieces (table, equation, rate of change, graph, etc. in any order that makes sense to them). This seems to be a good small group or individual task.
The graph I used is a first Quadrant graph since that lends itself to stories better. However, you can always delete it and drag in a four-Quadrant graph if that suits your needs better.
