We all know how difficult it can be to take data throughout the course of the day. All of our students have multiple and different self regulation goals on their IEP's and this is a perfect solution! Like many, my paraprofessionals are not trained or prepared to be as vigilant in taking data as we may like. I needed a system that got the basic information required to track their IEP progress, yet was simple enough to manage throughout the course of the day.
This data sheet provides classrooms with a simple, clearly defined way to monitor daily student behavior. All you need to do is individualize your student behavior information on the data sheet and then write a brief, easy to read, description of the target behavior. Then throughout the day, set a timer for once a hour (or however long you want your sampling to be) and then circle either yes or no on the data sheet if the student did or did not engage in the behavior.
This data sheet is easy to edit - simply find two target behaviors for each student that you would like to take data on, write a brief description at the top of each column for each student and print! You can also change the times, number of columns, etc.
