I use this to show the kids the things I'm looking for when I give them their daily points for participation and attitude.
To clarify: I don't fill one out for every student each day. I save some paper by using it only as needed. For example, I have my 1st period freshman/sophomore chorus...all do well, except for two students who show up late, with a bad attitude, and without materials. The rest of the students get full credit points, but those two students get their sheets filled out for the day. Then, at parent/teacher conferences, I am able to pull those sheets from my files and show parents exactly what it was that the students did to lose their points.
