Here is a template for both ensemble and private lesson teachers. It is in Microsoft Word so that you can modify it to fit your needs, unlike a pdf. It really helped my students and I hope it can help your students as well!
To use it, you can pre-fill the song title and measures/directions OR you can have your students fill in the information in class (lesson). It seems to be most beneficial to have the students fill in the information and directions because they tend to remember it better. It can also be used for method book studies and assigned scales or excerpts.
