My Data Notebook is something I've used the past few years with my third graders. They are able to come up with a new goal each quarter (also included are trimesters and semesters) and graph their personal data. Students reflect at the end of each quarter or term whether they've met their goal or not, how they met their goal, and what they can do to improve.
Goals can be academic, behavioral, work habits, etc. Each student will have a custom goal that he/she is working towards.
Data notebooks also work great alongside RTI. You have documented proof of nine weeks of data (if you're on a quarter system, like I am) to take to meetings.
Keeping data notebooks looks great on teacher evaluations! It shows students being responsible and accountable for their own learning!

-Ideas for assessments (choose something you're already doing! No need to come up with more work to fill out the data notebook. No one wants to assess for the sake of assessing!)
