This is a 12 page resource where students track their daily habits (physical activity, sleeping and eating) for either 3 or 5 days. They then evaluate this information in comparison to the standards for teens/pre-teens. Students look at factors that may be influencing their choices and decide on a goal to improve their health.

-A Math extension where students calculate the mean, median, mode and range of their physical activity and sleep data to aid them in comparing it to the standard (1 page printable)
