This questionnaire was designed to serve as a means of gauging student's attitude toward mathematics, computer knowledge, working with others and personal responsibility in the learning process. After some time I updated to this version for the purpose of data mining. The questionnaire was handed out to three sections of forty-two students taken Statistics in the same semester. The form and data set were used throughout the semester in lectures, lessons, workshops and projects.
