I created this template to help other Teachers in organizing their lesson plans. It is difficult when you are teaching a range of grades and a series of topics (k-12) in the course of a single day. I have included the following (please feel free to use it however you’d like!).
Page 1: A WEEKLY overview where you can write in the language and content objective for students based on subject area (Math/Reading/Writing/etc.). There is a small box where you can write in the Common Core State Standard (CCSS) that aligns with your lesson for the day. For Teachers in NYC, there is an area to write in the Target of Measurement (ToM) as well.
+ A detailed DAILY guide where you can write in the Grade, Unit, and date of the Lesson as well as the Content Objective, Language Objective, CCSS, and T.o.M.

+ List of students in Group based on Language Development (ideal for Push-in/Pull-out groups.)

Page 3: + A detailed DAILY layout for your lesson where you can write in your Warm-up/Review activity, Introduction, Presentation, Practice, Evaluation, Application/and or Closure, and Homework assignment.
Page 4 + Teaching Notes (for Literacy) where you may write in Theme Questions, Focus Questions, Decoding Skills/Phonemic Book Genre, a Literary Element/Text Feature Focus, and Content and Thinking Skill Targets.
Page 5: + An attendance sheet in which students can sign in (for records).
