Product Description
Included in this pack are nameplates (two on one page that can be printed multiple times) and two designs of a behavior chart. Each behavior chart has a place to list five goals. One chart has a square to mark for completion of those goals once for Monday-Friday. The other chart has a square for AM and PM, Monday-Friday.
Nameplates are approximately 10" by 3" with 8.5" in the name area for writing.

Helpful Hints:
-Print nameplates on cardstock, cut out, and laminate. Select to print the nameplate page (page 4) only and print out half as many copies as nameplates you need (since there are two on a page).
-Nameplates can be used to label other things around the classroom- not just name tags on desks!
-I have included two behavior charts: one with Monday-Friday labeled across the top, and one with Monday-Friday AM and PM labeled across the top.
o Monday-Friday can be used for: keeping track of homework, completing specific assignments, tracking turns at the computer, overall behavior for general students
o Monday-Friday am and pm can be used for: tracking behavior of students who need more intervention/rewards/praise/redirection, completing specific morning and afternoon assignments, keeping track of bathroom breaks, etc.
-Goals should be created by the teacher with the student
-In each square teachers can punch holes, place stickers, initial, give a 1-5 score, record tally marks, draw smiley/frowny faces, etc. to keep track of students meeting goals.
-The last section could be reserved for nightly parent signature
