This is a form students can use to summarize a newspaper article they have read. On the front it asks them to identify heading, subheading, byline, and dateline. It defines these words. It then asks to tell who, what, where and why about the article. On the back of the form they take the information they have gathered on the front and write a summary of the newspaper article. This is a great form for teaching students about reading informational articles and also being able to identify the different parts of a newspaper article.
