As a 7th grade ELA teacher I put the trust in my students to be honest about their reading progress.
At the beginning of each new independent reading genre, students receive a copy of this log. I also conduct a pace assessment so students can determine their weekly reading goal.
At the beginning of each class period, students simply record their starting page number in their book. They are surprisingly honest! I am able to quickly walk the room, see how each student is progressing, and conference if I am concerned. I also keep and project my own log so the students can always see how my reading is going. This provides motivation.
At the end of the 4 weeks, the time I give my students to complete at least one book (they can record multiple books on the same log), I collect and grade the logs. I count each blank as approximately 5 points and record it as a participation grade. It is then also sent home for parent signature. Of course if I had a concern over the month time frame, I would reach out to the parent earlier.
This log puts the responsibility on the student, not the parent and has greatly helped me in monitoring my students' reading progress.
