This is the note ID assessment sheet I use for the associated video which can be found on YouTube at the link below. All 15 note identification questions are displayed on screen after each level (3 levels in total). The YT video can be paused at any time for students who need additional time to identify. Although, at the end of the video all 15 notes are displayed for identification.

The Story of Inu...
Inu is an alien from a distant planet called Sonata, whose dream is to traverse the universe and share his music with everyone. He has a beautiful musical talent and a passion for music that is unparalleled. He visits many planets, performing shows for all the locals. Everywhere he goes, his music is celebrated and he is showered with love and admiration. The more he performs, the stronger his desire to keep going becomes.
Unfortunately, an evil emperor named Nocturno wanted Inu to be his personal musician. And after playing a show on Rondo - the emperor's home planet - Inu was captured and held as Nocturno's court musician. Inu, grief-stricken by the idea of being a servant to Nocturno, wanted nothing more than to break free and pursue his dream. So one night he escaped, and set off to continue his journey across the stars.
Meanwhile, Nocturno continues his desperate search for him, sending out bounty hunters, search parties and even enlisting the help of other planets to find Inu and bring him back to Rondo. But INU is determined to be free and evade capture. These are his adventures...
