Students learn more when they MAKE notes, not TAKE them. This form allows students to create a list of facts from a reading, video or other source, then create their own discussion questions for the class, write out their opinion to those discussion questions, and finally they need to select which facts support their opinion to their discussion items.
This has really helped my students to think about the media they are consuming. It has been described as Cornell Notes on steroids because of how fast it grows student understanding. This is a must have for any class!
