Product Description
This 6 page Word doc product is a great activity for students to complete as they received their term 1 report cards in February (Ontario). Students will be asked to thinking critically and honestly about their performance this term.

The bundle includes 2 pages for students to complete for this term, next term and the progress report later in the new school year (total of 6 pages). It is a table format and each page examines both the learning skills and subject area portion of the Ontario report card.

Subject area page includes all subjects: language, French, math, science, social studies (can edit for history / geography), health, physical education, drama (can edit for dance), music, visual arts.
Chart then asks the following:
- I got...
- Why did I get this?
- What I am going to do specifically to improve in this subject for next term

Learning skills pages includes all 6 areas: responsibility, organization, independent work, collaboration, initiative, self-regulation
Chart then asks the following:
- I got...
- Why I received this letter (S, N, G, E)
- What I am going to do specifically to improve this skill for next term

Personally, I have completed it in class with students, rather than send it home, as my goal is for students to think for themselves and be honest about what they did (or didn't do) to received the marks they did. Doing it in class also allows students to ask questions about their report card and get direct answers, as well as guidance when it comes to thinking about specific steps / actions they are going to take to improve their marks, learning skills levels. However, if you feel some (or all) of your students could benefit from completing this activity (certain sections or all) at home with parents, then you can do that as well. You know your students best!

Since it is in Word doc format, you can easily add, remove, and/or modify any aspect of this activity. This product can also be modified to suit the various needs (e.g. IEP modifications, accommodations, etc.) of your students.
