Description

Data tracking for therapists and teachers can be a tough task! Help to make it easier by using forms for observing and tracking data on MULTIPLE STUDENTS for RTI, full classroom, SPED, and self contained classrooms. This can also be used for RTI for screening all students for fine motor, visual motor, sensory processing and activities of daily living. This includes pre-writing, letters, scissor skills, manipulation of toys, locker management, clothing management and more!

  • Each form has 15 rows for 15 students. There are between 5 skills – 7 skills across the top of the page.
  • This includes:
    • Play skills / manipulation of toys (pages 3-4)
    • Scissor skills (5-6) – 1 sheet for tasks and 1 sheet for ergonomics

I am a school occupational therapist who has a doctorate in occupational therapy. I work closely with special education and general education staff adapting material. This is where I get my inspiration for this material. I have used my materials with a variety of children with different disabilities including learning disabilities, dyslexia, autism, sensory processing disorder, ADD, ADHD, visual impairments, deaf/hard of hearing, cerebral palsy, down syndrome and more.
