YOU CAN TYPE IN OR WRITE IN your data to TRACK your students' accommodations and the effectiveness of the accommodations!
It can be hard to document when accommodations are being utilized and if the accommodations are effective . This form makes it EASY to collect data and observational data in the student’s classroom to make sure accommodations are being offered and utilized. This is an accommodation tracker for 30 different common accommodations for visual motor, fine motor and sensory processing accommodations. There is room to write in accommodations that may not be listed, in both of the different boxes.
The top box has many accommodations listed and this should be filled out to the individual student to what the accommodations are currently being utilized. Once you fill this out, if writing - you can make a copy or if typing - save a copy, so you do not have to write in the accommodations each time when you fill up the sheet.
The boxes underneath with dates listed are when you are observing, so you can make sure the accommodations are being utilized.
