This is a tool to be used for students who struggle with self-monitoring and staying on task. Randomly or at timed intervals, ask the student if they are on task or off task. They answer what they think they were and mark it. At the end of the day, the tallies can be used to show the student if they spend most of their day on task or off task. Eventually, it can be worked to where they are in charge of asking themselves independently if they are on or off task and marking it honestly themselves. It is a great tool for students who are struggling with self awareness. If it is laminated, you can just clean it off at the end of the day and start new the next day, or even, if necessary, start new after lunch for students who extremely struggle with staying on task.
