Meeting with your students one-on-one can be very rewarding! They seem to open up and allow you get to know them better as readers and writers. I use the pages in my classroom to jot down notes about our visits. On both the reading and writing conference pages I leave room for a "glow" and a "grow." Glow: something the student did well. Grow: A goal to work on until the next time you meet. These really help me remember and keep track of skills that my friends are working on and ones they already can do!
