Do you have math centres and need to keep track of students' activity, learning, conversations, and assessment?
I have several copies of these checklists - one for me, my peer tutor, parent volunteer, EA, etc.. When we observe the student completing a task (independently), we use a marker to document whether the student can't do it on his/her own yet (red), can somewhat do it (yellow), and is completing the task independently (green). Older observation dots on the left of the column, newer (more recently) on the right. I like to aim to have at least 2 green dots in a row to confirm the student has "mastered" that skill or knowledge. Alternatively, you can write the observation date and highlight in the appropriate colour.
These are the expectations I use to assign a mark for and comment on Number Sense and Numeration (Money, Coins, and Skip Counting).
