I use this throughout the year in order to facilitate parent-teacher communication. At the end of every month, I get students to reflect and self-evaluate their learning skills in one column of the document (student column), then I assess them in the teacher column, and then the parents can sign, comment, and get their kids to return the booklet to school for the next month.
I find that this really helps keep students on track and parents informed. This way there are never any surprises on the report card. It also helps track learning skills efficiently from month to month. All of the month are included in this booklet- except for the month of June because that's when students get their final report cards.
The descriptors that I have used for each learning skill is taken directly off of the Ontario Provincial Report cards and this way both students and parents know exactly what they are striving for month after month.
