This form is to be used in conjunction with behavior plans already in place in the classroom. Students can use this form to think through poor behavior choices write out the action and what they can do differently next time.
Teachers then can discuss the form with students privately and write a short note to expand for parents what occurred and what was discussed. The form can be taken home to be shared with parents and signed.
- Has the student thoroughly think through the issue and point out what was done wrong. Think through what they specifically could do next time
- Gives the teacher the opportunity to discuss with the student the behavior and their future choices
- Allows the teacher to communicate with the parent clearly what occurred
- Encourages Home-School communication
