This missing/forgot homework form is great for helping students learn responsibility, keeping track of missing assignments, and keeping in touch with parents.
I have used this form in my classroom for several years now, and have gotten lots of positive feedback from parents. My students know where the Oops! sheets are, and at the beginning of the year, they learn that if they do not do their homework for some reason, it is their responsibility to fill out a slip. They must fill the form out completely, take it home, and get it signed by a parent.
This allows students to learn responsibility; it is up to them to fill out the slip completely, including checking off an explanation for missing their homework. (This section of the form is particularly helpful for the teacher. It enables you to learn things that your students might not have shared with you otherwise.) Additionally, having a parent signature is proof that the parent is aware that students have not completed homework. I always save these slips for documentation purposes.
