I use these forms to notify parents of unwanted behavior. I use a frog system. When three frogs have been pulled in a day, the child receives an Oops Note. I call it an Oops note because I teach my kids that everyone makes mistakes. Just because they made mistakes today, they can always do better. We discuss what could have been done differently and how they will try harder the next day. These forms provide me with documentation if needed for parent conferences, etc.
