Some students fail to use passing time properly; they wait until they get into the classroom to ask to leave the room for supplies or a restroom break. Oops passes are distributed to each student at the beginning of each quarter (or week, or any time period). Distribute a specific amount to each student, such as three per quarter. Students must use those passes when they need to leave the room (when they run out, they can't leave unless its an emergency). They can also be modified to include being able to use the oops pass to borrow supplies, such as a pencil, from the teacher.
