This is an oral reading fluency chart that I made to help me keep track of each student’s reading progress throughout the year. I use it in conjunction with the Treasures literacy program put out by Macmillan/McGraw-Hill, although you really could use it with any reading program.
For use with the Treasures program, I have each student read the weekly timed reading selection with either me or a parent volunteer each week. Depending on their reading level, the student will either read from the approaching, on level, or beyond level reading selection. After the student reads, I (or my parent helper) will fill out the chart to keep track of how fluently he or she reads their 1-minute timed selection for that week. I use the same chart for an entire unit’s scores for each child.
