This resource will help you establish communication with your students' families and will serve as a foundation for your relationship with parents and/or guardians.
The form enables you to introduce yourself, your team, and your school. The editable forms include a section to provide information including your classroom location, office hours, and email address, as well as the names of any colleagues that work within your classroom.
This form will also prompt parents and/or guardians to provide their contact information and preferred methods of communication. Additionally, parents and/or guardians will have the opportunity to share any important information about their child.
This resource provides a simple approach to documenting Parent-Teacher Conferences. Each log binder is designed for a single class of up to 34 students; if you have more than one class, you can print multiple logs and assign one for each section that you teach.
For paper-based learning, the printable templates include space for a variety of discussion points. Most importantly, each template provides space to document your conversation with each parent, guardian, and/or student.
This resource also includes a digital log binder for Google Slides®; this format is convenient as it allows you to organize your log using tabs for each student, and link each tab to the student's individual page. Another advantage to using Google Slides® is the ability to hyperlink to anecdotal notes and student work.
The Google Slides® document includes both editable templates and fixed backgrounds to meet the individual needs of your classroom. These templates include options for student-led conferences, areas of strength and growth, student work, and student comments.
This spreadsheet will help organize anecdotal notes, attendance, outstanding assignments, and parental correspondence for each student in your class.
This Google Drive® spreadsheet includes a section to document concise anecdotal notes. The intention of this spreadsheet is to keep a detailed overview of each student, with a collection of data all in one place. For more detailed notes regarding a student observation (including incident reports or narrative anecdotal notes), use the editable Detailed Anecdotal Notes Template and hyperlink the document into the spreadsheet.
For each anecdotal note, there is a corresponding section to note any communication or follow-up with a parent or guardian. You can note the date of correspondence, as well as any notes from your conversation.
Additionally, this spreadsheet includes a section to note attendance, including both excused and unexcused absences and late arrivals. This will allow you to focus on anecdotal observations while simultaneously having a record of the students' attendance for cross-checking and further analysis.
Finally, this documentation record also includes a section for outstanding assignments. You can note both the date the assignment was due, whether or not the assignment was eventually turned in, and the date on which the assignment was submitted.
