This is a worksheet to give to students that need to make-up a missed day of PE. This is a very basic worksheet that is meant to place the majority of the emphasis on the actual physical participation that was missed, and this worksheet allows the student to document the physical activity and for the teacher to be able to fairly assign a grade.
The students are instructed to participate in an activity that will improve their overall fitness. (Example: bike riding, walking, tennis, swimming, dance lessons, etc.) They should complete a minimum of 30 minutes of exercise.
They are then instructed to answer multiple questions, as well as write a reflection.
