Who: This activity is meant for use by group members in a Professional Learning Community (PLC).

What: A Problem of Practice (PoP) is an area in need of focus. PLC members can use this action planning sheet to record their PoPs/focus areas, action steps/strategies for intervention, and eventual outcomes.

Where: This activity can be used in any PLC.

When: This activity is a planning tool so, although it can be used at any point during the school year, it should be used at the beginning of a new action plan/inquiry cycle.

Why: In order for change to occur, educators must identify those areas in need of change. This worksheet helps PLC members narrow down their areas of focus while coming up with distinct steps for making positive change. It also serves as a reflection tool so PLC members can change or add to their action plan.
