This is a form to use with parents during your Parent-Teacher Conferences. It is set up as a checklist of classroom skills/standards. It is simple to fill out and written in language your parents will understand and appreciate! You can prepare it before your parent conferences or I have also completed is as we discuss each area. I have found it keeps me focused and on schedule and it ensures I am covering everything I need to during those short conferences! As well, it is a great takeaway for parents to share with their child at home. There is a place for comments and there are even suggestions for websites and home resources to try for help in weak areas.
Tip: I usually fill these out before the conference and make TWO copies to bring to conferences. One is for the parent, and one is for me to jot down any notes or plans that came up during the conference so I have a nice record of what we discussed.
