One of the hardest things to manage as a teacher is all of the parent contacts you should be making, and when students are failing your class, it can be time-consuming to write out individual behaviors you see that affect student performance, ways that the parent can help you, and what the student needs to do to improve. It can be difficult to express everything required for the student to be successful.
This is a form designed to help you quickly and efficiently make contacts home to parents. This form includes a checklist of common behaviors that get in the way of student performance, as well as conversation starters for how parents can discuss their behavioral and academic issues with their child.
