This is the form I am most excited about! When a parent emails or calls (or even stops me in the hall), I scramble trying to find what I said last to him/her. I've made it my mission to get more organized this year, and I hope this will help you too!
There are multiple sections on this two page sheet-basic information (so you can recall who you spoke to, when, and how), a section for you to list the concerns or praise that had you reach out in the first place (or the parent's concern), a section for your notes as you are conversing with the parent, a follow up section (so you know whether you need to follow up and then a box to check off when you do), and a section for action items (teacher and parent).
I plan on printing multiple copies front to back and storing them in my new Parent Contact pink binder! :-) You could put in tabs for each student, and move one of these sheets behind that tab as you fill them out, as well. Some people choose to have a hanging folder, as opposed to a binder, for each student. You could place this sheet in the folder as you finish. The possibilities are endless, but you will have all the important information from each contact at your fingertips!
